Medical Treatment Authorization

In the event of illness or injury, I authorize Be Discovered staff or representatives to obtain emergency medical treatment for the participant if deemed necessary. I understand and agree that I am responsible for any costs associated with such treatment.

Liability Release

I understand that participation in the Be Discovered program, including rehearsals, classes, performances, travel, and related activities, involves inherent risks, including but not limited to bodily injury, illness, disability, property damage, or death. I knowingly and voluntarily assume all such risks associated with participation. On behalf of myself and my heirs, executors, administrators, assigns, and representatives, I release, waive, and discharge Be Discovered LLC, its directors, officers, employees, instructors, contractors, assistants, volunteers, agents, and representatives from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation in the program, including those resulting from the negligence of the released parties to the fullest extent permitted by law.

Program Changes and Force Majeure

Be Discovered LLC reserves the right to modify or alter the Program as deemed necessary. Be Discovered LLC shall not be held responsible for delays, cancellations, or changes due to circumstances beyond its control, including but not limited to acts of God, weather, pandemics, government restrictions, transportation disruptions, venue issues, labor disputes, illness of instructors or staff, or other unforeseen events. Be Discovered LLC may adjust schedules, locations, instructors, performances, or program activities as needed. In the event of cancellation or date changes, participants will be offered the following options (i) reschedule or participate in another available program, or (ii) transfer registration to the following year.

Media / Video Release

I grant permission to Be Discovered LLC to photograph, film, record, and otherwise capture my name, image, likeness, voice, and participation during program activities. I authorize Be Discovered LLC to use, reproduce, publish, distribute, and display such materials in any media, now known or later developed, for promotional, educational, marketing, or commercial purposes worldwide and in perpetuity, without further compensation or approval.

By submitting this agreement, the participant acknowledges that they understand the terms of this agreement and agree to be bound by them, and confirms that the information provided in this agreement is accurate.
